Our Intake Form takes around 5 minutes to complete.
This form will also be emailed to you the day before your scheduled appointment.

If your form is not completed by the time of your appointment starts, filling this out in-studio will affect your appointment duration and may result in your Esthetician needing to reschedule your appointment.
